47 Year Old Female – Shart Shooting Pain medial ankle, navicular area, upon initial stance.

Hi Everyone, This is the first time posting to Pod Arena, however, this case is slightly puzzling and inconsistent in many aspects. I would just love some fresh eyes on it. 47 year old female presented to our podiatry clinic a few months ago with a log history of pain in her left medial ankle/ posterior tibial tendon region/ navicular region.
