Hi There, im relatively new to this but am finding out loads from reading all these threads and posts that I thought I’d post my own. I am currently about to undertake a clinical audit following recent introduction of using the SOAP documentation protocol in our department. I am wondering if anyone can help me find articles supporting and explaining the rationale for utilising SOAP format
